World 13 is an underground crystal cavern world resembling Tilted Tunnel from New Super Mario Bros. U in Dr. Mario World. The first area is available as of 1 AM PT on Mar. 5, 2020[1], while the second area, challenge stages and special stage are available as of 2 AM PT on Mar. 19, 2020[2].

When an area is cleared, Bony Beetles, Bone Goombas, and Swoops appear. The viruses featured in the regular stages are red, yellow and blue, with the exception of one stage, which has green viruses in addition to blue.

  2. The 20 coins surrounding the barrels.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

Starting with 33 capsules, the objective of this stage is to eliminate all viruses (29) and break 17 Brick Blocks.

In this stage, fully-frozen Brick Blocks form a boundary that surrounds the viruses in it. There are viruses outside the boundary that are also next to bubble machines. Pop cannons are at the corners of the Brick Blocks.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is reduced from 34 to 33.
  • The first and last Brick Blocks in row #8 are replaced with pop cannons.
  • The objects between the leftmost and rightmost Empty Block in the topmost row are changed from 6 ice fans with an Empty Block in between them to 5 fully-frozen Brick Blocks in the middle. As a result of having more Brick Blocks than the objective requires, not all of them need to be broken.

Starting with 38 capsules, the objective of this stage is to eliminate all viruses (18), collect all coins from Coin Blocks (27) and break all crates (36). The only colors available in this stage are red and blue.

In this stage, Brick Blocks each containing coins are found between floatie viruses at the top and bottom in groups of columns of three, which are all between crates in groups of three in a column in a cascading pattern. Exploders are found outside the group next to crates.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 37 to 38.
  • The available colors are changed to red and blue only.
  • All Empty Blocks are removed in this stage. In their place, exploders are added in where the middle Empty Block of each group used to be.
  • All viruses are replaced with floatie viruses.

In this stage, 3 shells are found in the middle column, where they are aligned with lock blocks, floatie viruses and purple coins. Clearing the blue lock block unlocks cages containing viruses in the rows containing Empty Blocks while clearing the red lock block unlocks the cages containing Brick Blocks containing coins.

Purple coins are unlocked in the following sequence:

  1. The 4 coins in rows #4 and #6.
  2. The leftmost and rightmost coins in rows #3 and #7, for a total of 4 coins.
  3. The rest of the coins in rows #3 and #7, for a total of 8 coins.
  4. The 12 coins in rows #2 and #8.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 39 to 40.
  • All Empty Blocks are removed in this stage.
  • A shell is added in the middle of row #5.

The last stage of the first area and starting with 36 capsules, the objective of this stage is to eliminate all viruses (17) and collect all purple coins (12).

In this stage, all colorful crates are wrapped in bubbles, with several of them floated upwards toward viruses. The initial three rows and the three rows after that also contain half-frozen coins and dust viruses. Brick Blocks do not contain anything.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced.

Purple coins are unlocked in the following sequence:

  1. The coin in row #3.
  2. The coin in row #2.
  3. The coin in row #1.
  4. The coins in the next three revealed rows, each revealed row containing one coin for a total of 3 coins.
  5. The 6 coins in the sixth-revealed row.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain the Bony Beetle assistant upon completing this stage.

In this stage, Brick Blocks form a boundary that splits the stage into four quadrants, with the bottom quadrants containing barrels, all of them containing exploders. An exploder is located in between the bottom row of barrels. Four keys are found, with their respective doors in the topmost row of Brick Blocks. Flasks are hidden in the Brick Blocks in the leftmost and rightmost columns in rows #5 and #9, and an exploder is hidden in a Brick Block in row #4 of the middle column. Viruses and floatie viruses are hidden in the other Brick Blocks. The filled flasks in this stage will shoot out either empty Brick Blocks or Brick Blocks with hidden viruses.

In the final stage of this world and starting with 37 capsules, the objective is to eliminate all viruses (61) and collect all keys (3).

In this stage, the middle column contains viruses, keys and lock blocks inside bubbles. On the left and right sides in the beginning, crates, a few Empty Blocks and a few half-frozen bubble machines form a path containing viruses. After the stage scrolled up far enough, there exploders and doors in cages, along with several viruses above Empty Blocks.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 14. Upon completion, players earn a free Staff Ticket.

  3. The 3 coins in row #6.
  4. The 3 coins in row #5.
  5. The 3 coins in row #4.
  6. The 3 coins in row #3.
  7. The 3 coins in row #2.

Completing this stage awards the player 500 coins.

In this stage, the ring of barrels contain viruses, and the ring of dust contain barriers. Barriers in dust will not switch on or off. All Brick Blocks contain coins.

Completing this stage awards the player 1000 coins and a Staff Ticket.

Starting with 45 capsules, the objective of this stage is to eliminate all viruses (13) and collect all keys (35). Unlike the rest of the special stages which have the available colors of pink, blue and yellow, the colors available in this stage are red, blue and yellow.

In this stage, keys are blocked by different things before the doors. In the lower rows, the doors are half-frozen. The following rows have hidden doors in Brick Blocks. Colorful crates are between the keys and doors in the next rows. The following rows have barriers between the keys and doors. Near the top of the stage, doors are locked behind cages with its corresponding lock block at the corner.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced.

Completing this stage awards the player 1000 coins and Dr. Fire Luigi.
