World 12 is a candy-themed world in Dr. Mario World. The first area is available as of 2 AM PT on Feb. 3, 2020[1], while the second area, challenge stages and special stage are available as of 11 PM PT on Feb. 19, 2020[2].

When an area is cleared, Ant Troopers and Stingbies appear. The viruses featured in the regular stages are red, green and light-blue.

Capsule Plus numbers can decrease even though it is frozen. A shell is in row #9 with two pop cannons, and an exploder is located in the middle of row #3.

The capsule amount is increased from 22 to 25 since version 2.1.0. Besides that, this stage is otherwise the same.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • The first and middle viruses in row #1 are replaced with shells (the middle one still being fully frozen).
  • The middle virus in row #3 is replaced with an exploder.

Starting with 15 capsules, the objective of this stage is to eliminate all viruses (4) (including any that appear from flasks before this objective is achieved) and break all Brick Blocks (36).

This stage contains Brick Blocks next to viruses and Capsule Plus capsules, where the Brick Blocks found in the top six rows are half-frozen. The filled flask in this stage will shoot out either viruses or Capsule Plus.

Exploders are found surrounded by crates and cages. The filled flasks in this stage shoot out either exploders or floatie viruses.

The last stage of the first area and starting with 50 capsules, the objective of this stage is to eliminate all viruses (67) (including any that appear from flasks before this objective is achieved) and collect all keys (7).

In this stage, the bottom four rows contain rows of viruses (normal, within barrels, half-frozen) with a gap in between each of them, and the five rows above it have viruses contained in all undamaged barrels and most Brick Blocks. Two keys and three exploders are found in a few of the Brick Blocks. Followed by this, the five undamaged barrels in a row all contain keys, along with six doors directly above them.

In the section above the doors, colorful crates are arranged in a pattern where floatie viruses and Capsule Plus are found in the larger gaps, while the lock blocks are found in the smaller gaps that unlock the colorful crates in the leftmost column, which are located below the door. The filled flasks in this stage will shoot out either viruses or viruses shrouded in dust.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 11 lines are introduced.

When cleared, the rest of the first area in the world map gets cleared of viruses. Players also obtain the Ant Trooper assistant upon completing this stage.

Prior to version 2.1.0, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 40 to 43.
  • The stage is shifted up by one row, resulting in the scrolling lasting for 11 rows instead of 10 rows.

The capsule amount is increased from 43 to 50 since June 2021. Besides that, this stage is otherwise the same.

Two rows of four viruses are confined with one side open, with one side in Empty Blocks and one side with both Empty Blocks and crates.

Prior to version 2.1.0, the stage layout was different. The following changes were made:

  • Capsule amount is increased from 40 to 43.
  • The crates in the eighth row have reduced durability (4 hits to 3 hits) while the remaining crate instead have greatly reduced durability (4 hits to 1 hit)

In the final stage of this world and starting with 40 capsules, the objective is to eliminate all viruses (30) and collect all purple coins (13).

In the beginning portion of this stage, barrels are found next to fully-frozen viruses, each containing colorful crates. Above the section with barrels are lines of viruses between colorful crates occupying two rows. Above that are two rows containing lines of floatie viruses between crates. In the section above, purple coins and viruses are found between colorful crates. More purple coins are found two rows above that section.

Purple coins are unlocked in the following sequence:

  1. The 3 coins in the first-revealed row.
  2. The 3 coins in the second-revealed row.
  3. The 3 coins in the third-revealed row.
  4. The 4 coins in the sixth-revealed row.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 8 lines are introduced.

Once it is cleared, the remaining area of the stage map gets cleared of viruses, and players can access World 13. Upon completion, players earn a free Staff Ticket.

Prior to version 2.1.0, the stage layout was different. The following changes were made:

  • The stage is shifted up by one row, resulting in the scrolling lasting for 8 rows instead of 7 rows.

Completing this stage awards the player 500 coins.

The timer is increased from 70 seconds to 80 seconds since version 2.1.0. Besides that, this stage is otherwise the same.

Within an 85 second time limit, the objective of this stage is to eliminate all viruses (62) and break all colorful crates (55). The colors available in this stage are green, light-blue, and pink.

In this stage, colorful crates have lines of viruses (horizontally and vertically) next to them. Colorful crates do not need a match of the same color to remove the ice, but still require a match of the same color to break. Additionally, a key is found in the middle column, which will be used to unlock a door between a row of fully-frozen green viruses. A row of fully-frozen colorful crates are above the aforementioned row. In the last two revealed rows, half-frozen colorful crates with like-colored viruses in between are found in two of the rows above it.

This stage will scroll for one line each time enough objects are cleared or enough capsules are added to the stage and will stop after 7 lines are introduced.

Completing this stage awards the player 1000 coins and a Staff Ticket.

The timer is increased from 80 seconds to 85 seconds since version 2.1.0. Besides that, this stage is otherwise the same.

Prior to March 18, 2021, the stage layout was different. The following changes were made:

  • The timer is reduced from 85 seconds to 80 seconds.
  • The colorful crates in rows #1, #2, #6 and each of the next three revealed rows are no longer frozen.
  • The viruses in row #3 are changed to become fully-frozen.
  • The colorful crates in the sixth- and seventh-revealed row are changed to become half-frozen, and like-colored viruses are added in between those colorful crates.
  • The row of Empty Blocks are removed from the topmost row, rendering the stage with only 7 rows to scroll through instead of 8.

Completing this stage awards the player 1000 coins and Dr. Larry.
